Paperwork Checklist

Please find all the necessary paperwork below. If a document is not linked, this means that this form must be completed through your online dashboard at https://amigos.my.site.com/Apply/s/login/

Part I Paperwork due February 1st:

• Consent and Release and Travel Forms (PRIORITY)
  • If under 18 at time of project start:
    • Consent and Release Agreement and Consent for Travel of Minor & Power of Attorney
      
      • Notary required
  • If 18 or over at time of project start:
    • Consent and Release Agreement and Medical Power of Attorney
• Confidential Physical Health Form  (PRIORITY)
  • Immunizations Record - submitted with Physical Health Form. See immunizations list here!
  • Confidential Mental & Behavioral Health Form (applicable only to volunteers who are seeing or have, in the past two years, seen a treating mental health clinician)
• Health Insurance Information (PRIORITY)
• Typhoid Immunization (PRIORITY)
• Personal Information Form
• Passport Information Form (must have valid passport ready)

 Part II Paperwork due April 1st:  

• Pre-Travel Health Confirmation   (will be available on March 1st) 
• Background Check & Criminal Record Affidavit
• Rooming Preference Form
• Volunteer & Family Questionnaire - parents should complete this for volunteers under 18; volunteers 18+ can complete this for themselves) 

Late Paperwork Fee

Late Paperwork Fee  - By April 15th

If all paperwork is not received by the priority deadline, a $250 late paperwork fee will be applied to your statement on April 15th. Unless volunteers were given a deadline extension by Admissions.

*Paperwork deadline is April 15th for volunteers accepted before April 1st. Volunteers accepted after April 1st have 30 days or less to complete onboarding paperwork (to be communicated by Admissions upon acceptance), depending on proximity to project start.

Late Paperwork Fee  - By June 1st  

If all paperwork is not completed by June 1st, a $500 final late paperwork fee will be applied to your statement. For volunteers accepted after April 15th, this fee applies if any required forms, including additional health forms, remain missing by June 1st. Late onboarding volunteers who miss the revised deadline communicated by Admissions will also be assessed this fee. Unless volunteers were given a deadline extension by Admissions.

*This increased fee accounts for the additional resources required for late clearance, including on-call medical screening, to support the Health and Safety team in safely clearing volunteers for their projects.
